5. DEPENDENT OR MINOR CHILDREN
The wife is not pregnant.
There are two minor or dependent children. The children are: A.G., a female, born 8/25/2002 and E.G., a male, born April 29, 2010.

Petitioner requests that this Honorable Court enter an order dissolving the marriage, and enter an Order establishing sole parental responsibility in favor of the wife, award the wife child support, and award any other relief the Court deems necessary and just.
I understand that I am swearing under oath to the truthfulness of the claims made in this affidavit and that the punishment for knowingly making false statements includes fines and/or imprisonment.
